It's basically October, when everything turns pink and we're reminded to "get our mammograms." But this year, I want to share a perspective that challenges how we should approach screening: what does actually smart, safe screening look like?

I recently listened to Dr. Jenn Simmons (a former breast surgeon turned integrative oncologist) on the Primally Pure podcast, and her message is simple but powerful: "We cannot expose healthy tissue to toxins. Can't expose them to radiation. Can't expose them to gadolinium. It's the wrong thing to do."

She's not anti-screening—she's pro-smarter screening that avoids unnecessary risks. Understanding what safer, more effective screening means can empower us all to make informed choices.

Start With Awareness, Not Conventional Imaging

Mammograms and MRIs find structural changes that are already present. But because of breast density, growth patterns, and timing, they're not always the ideal first step.

Dr. Simmons recommends a smarter screening strategy:

1. Start with self-awareness — Monthly breast self-exams where you get to know your normal and watch for changes.

3. Save conventional imaging for follow-up — Use mammograms or MRIs with contrast only when steps 1-2 show something that needs further investigation.

The Nuance We Need to Understand

Long-term studies show that mass mammography detects more breast cancers—but also detects lesions that may never have progressed. This is called overdiagnosis, and 20-30% of people diagnosed through mammography may never have developed clinical disease.

Even Dr. Otis Brawley (former Chief Medical Officer of the American Cancer Society) has said: mammography has benefits, but it's not perfect, and we can't oversell what it can do.

Here's what the research shows: annual screening from ages 40-74 may induce 125 breast cancers per 100,000 women screened (from radiation exposure), while preventing 968 breast cancer deaths. That's still a net benefit—but here's Dr. Jenn's question: Could we save those same 968 lives WITHOUT causing 125 cancers if we used smarter, radiation-free screening strategies instead?

Plus, a Swedish study found that women who had false-positive mammograms were at higher long-term risk—the false positive revealed underlying risk factors that were already present, but no one followed up to address them.
